Agood alloy welder could repair it but engine would need to be stripped .
Just give it a little bit exrta on valve clearance especially exhaust, at worst it will rattle a little.

Valve clearance is set regardless of CC, the exhaust clearance is always slightly bigger due to it getting hotter, expanding and making gap...
It has the potential to flap around and jump off the teeth on crank gear, it's only a 5 min job to fit.
Possible, clean bendix with petrol or carb cleaner then blow out with hp air and don't put oil in it, it needs to be bone dry !
Take transmission cover off, try to turn engine over.

That looks like the minerelli engine to me, not Piaggio !
Early Piaggio's had an alloy flange but later are all black, still have the alloy flange under the rubber coating.
In the scooter world it's very old. check the reed petals to make sure of no cracks or not closeing properly. I'm sure any piaggio 50cc inlet...
I hav'nt got my Haynes manual to hand, check all fuses.
