ive seen it loads of times, its either: loose primary nut, badly fitted outer pully, bad crank, bad crank splines, over tightened primary nut or the casing has a defect (ei the kick start mech is put on wrong) most common of these would be someone taking the restrictor out and putting it together wrong/wrong torque on the nut....
