Hi I have a rolling nut typhoon 125 engine I have just replaced the hub bearing and seal for mot and it still failed, any idea?

Also just brought a STR8 stem and no matter how much I tighten the bolt down in the middle that goes down to the metal that goes in the forks it just won't grip and when I hit the lock stops the stem turns

For the MOT - no Idea. For the stem - make a shim out of a beer can and put it inside the neck of the stem before putting it on the shaft. Cut to size so it fits the 360 inside of the stem. That should do the trick. If one isn't enough - make a longer one that goes 720.

Just make sure it fits mega tight as it's the only way to control your bike. If it fails your fucked and are likely to die in pain.
